South Riverdale Community Health Centre

Doctors, nurse practitioners, health professionals and case coordinators serve patients who have difficulty accessing health services because of homelessness, drug use and/or mental health problems. The team offers primary health care, as well as information and referrals to other community services. Services are offered through our outreach clinic, as well as shelters and other community programs. This is a walk-in clinic, clients may have to wait before they are seen by a doctor or nurse practitioner.
