Cardiac Care Network

As a dynamic partnership between professional providers, institutions, community members and government, providing advice and system supports based on evidence, best practices and data analysis, CCN is an essential resource in improving the delivery of adult cardiovascular care in Ontario. The CCN is a support system to the Ministry of Health and Long-Term Care dedicated to improving quality, efficiency, access and equity in the delivery of the continuum of cardiac services in Ontario. Using data and consensus-driven methods, we offer planning advice for the future of cardiac services and the provision of exemplary care in collaboration with the Ministry and others.
