Humber River Family Health Team

Welcome to the Humber River Family Health Team (HRFHT) web site. Our goal for this site is to better inform our patients and the community about the programs and services offered by the Humber River FHT as well as our upcoming events, history, and objectives. The HRFHT is situated among Toronto's most culturally diverse populations. It is in the catchment community of the Humber River Hospital, an area encompassed by Steeles Avenue to the north, Kipling to the west, Eglinton to the south, and Bathurst to the east.
