Forward Care Medical Clinic

Forward Care Medical Clinic (FCM) is one of the first clinics of its kind in the Lower Mainland. We operate on a shared care model between specialists and general practitioners in order to help provide comprehensive care to an aging population. We currently have five family doctors and three internal medicine doctors specializing in endocrinology, geriatric care, and cardiology. To effectively address these issues and concerns, we at FCM aim to provide a 'medical home' where all your care plans are kept in one centralized electronic medical record.
